Check-In - All Teams Must Check-In in the Cafeteria Before Playing

Please plan to arrive 30 minutes before your first game. Park in the south lot at American Fork High School. A Midnight Madness banner will mark the main entrance for the tournament. After entering the building please go straight to the cafeteria (on your left) to check your team in and pick up wristbands.

Wristbands

All players on your team must wear their wristbands for the entire tournament. Referees will check for them before each game and only those with a team wristband will be allowed to play. Food

For your convenience, we will have an awesome selection of sandwiches, pizza, snacks, treats and drinks for purchase in the cafeteria and the Cave. *Only credit cards, debit and Apple Pay will be accepted- no cash.

IF YOU ARE REGISTERING YOUR TEAM, YOU WILL BE CONSIDERED THE TEAM ORGANIZER/CAPTAIN & POINT OF CONTACT. YOU WILL BE RESPONSIBLE FOR RELAYING ALL RULES AND EXPECTATIONS TO ALL PLAYERS ON YOUR TEAM.

ZERO TOLERANCE POLICY WITH FIGHTING AND/OR RETALIATION, INCLUDING PARENTS

Game Details:

Games will be 5v5 including goalkeepers.
All teams are guaranteed a minimum of 3 games.
Each game will have two 10 minute halves.
Playoffs will be held after group play for each bracket.

Rules:

*Teams with more than 2 returning high school players should play in the competitive division. This applies to all high schools.

*No more than 3 returning AFHS players may play on the same team.

*Divisions are coed optional. Both boys and girls may be on each team, but it is not required.

All high school division players must be currently enrolled in a school. High school alumni may not participate in the 9-12th Grade divisions unless they are currently playing on a 2005/06 team and under 19 years of age. Any team found to have one or more non-high school age participants or players will be immediately disqualified and forfeit all remaining matches. Any team found violating this rule will be immediately disqualified and removed from the tournament.

Any team that displays violent conduct, fighting, or attempted fighting will be immediately disqualified and removed from the tournament.

Only one person per team needs to register. Do not register every player on the team. 1 Registration = 1 Team. Whoever registers the team and pays the team fee is the team captain and point of reference throughout the tournament.

By registering, all participants and all players on all teams agree to the rules & waiver below.

WAIVER:

AFHS Midnight Madness @ The Cave 5v5 Futsal Tournament WAIVER: I as a participant or parent/guardian of the participant I hereby assume the entire risk and hazards incidental to the conduct of the activity of futsal as well as the transportation to and from the activity. I release, absolve, and indemnify AFHS soccer and AFHS, employees, volunteers, and/or sponsors from all risks and hazards associated with the activity in the event of an injury. I do expressly waive all claims against them. I understand that the AFHS soccer program and AFHS provide no individual insurance coverage. As a parent in this program, I recognize and acknowledge there are certain risks of physical injury and agree to assume the full risk of any injuries, damages, or loss which my child may sustain, as a result of participation in such program as against American Fork High School, Alpine School District, their officer’s agents, servants, and employees. I do hereby release and discharge all the above-mentioned entities from any and all claims from injuries, damages, or loss which may occur to my child from participation in this program/activity. I further indemnify, hold harmless, and agree to defend the above-mentioned entities and their officer’s agents, servants, and employees from any and all claims resulting from injury, damages, and losses sustained to my child arising out of, connected with, or in any way associated with activities of this program/activity. This waiver is also an official release for the AFHS Midnight Madness @ The Cave 5v5 Futsal Tournament to take and display pictures of participants and of this event as they deem necessary to further market the event. All pictures may be displayed on the website and other marketing tools or platforms.
