This $100 Fee for our 9th to 12th Grade Student Athletes is essential for us to provide state-of-the-art, safe equipment for competitive play. Payment is required before uniforms are distributed. I acknowledge that the participant for which I am registering will participate in this event hosted by the school. I release the school and district, its agents, employees, representatives, and volunteers from any liability for injury, death, or property damage that may occur during the event. I agree to indemnify and hold harmless from any such claims arising from the participant's actions during the event. I have read and understand this release of liability, and I agree to it freely and voluntarily. I certify my ability to do so on behalf of the participant.
