Liability Waiver: In and for consideration of my daughter's/son's participation, I hereby agree and promise that I will not hold Natrona County School District, nor it's employees, responsible for any loss, damage, or personal injuries that she/he may receive as a result of participation. I am aware and understand that insurance is NOT provided and any medical bills incurred from injury while participating in the clinic are the sole responsibility of the individual and her/her family.
