I hereby give consent for my child to participate in Pasadena Memorial High School (PMHS) Softball Camp. I understand that injuries and property loss can occur during such a sport as softball. I hereby release and agree to hold harmless PMHS, coaches/instructors, and/or affiliated companies and their respective agents and any employees of PMHS of any liability claim, demands, injuries, or causes of action which may arise by virtue of acts, or decisions to act, negligible or otherwise. Pasadena Independent School District does not carry insurance for summer fitness/recreation programs. Parents will be responsible for any medical expense incurred. A parent/guardian MUST sign this form if child is under 18 years of age. The undersigned parent/guardian authorizes PMHS, coaches/instructors, and employee’s permission to request medical treatment as necessary to insure the well-being of the above listed dependent.
