I have attended bootcamps with Dr. Sample or Dr. Bush or plan to the week of my test.

*I understand and agree to the following:

I understand that I MUST wear my CHS school badge for testing so that my ID can be verified by the school test administrator.
I understand that I will not be allowed to enter the testing room and take this exam if I do not bring a fully charged Chromebook AND charger with me!
I understand that I am responsible for transportation to and from the testing space.
I understand that standard testing procedures will be followed in relation to testing security, meaning I do not need to bring anything to the testing space with that is not necessary. I will turn in my phone and other belongings as I enter the room.
