Delivery Instructions

Each participant will receive 2 shirts. One for participating in the event and the other to wear in the parade. These will be handed out the week of the clinics.

I acknowledge that the participant for which I am registering will participate in this event hosted by the school. I release the school and district, employees, representatives, and volunteers from any liability for injury, death, or property damage that may occur during the event. I agree to indemnify and hold harmless from any such claims arising from the participant's actions during the event. I have read and understand this release of liability, and I agree to it freely and voluntarily. I certify my ability to do so on behalf of the participant.

We are needed the following Measurements:

A - Chest - Measure around the body at the fullest part of the chest, under the arms, and with the arms down and relaxed.

B - Waist - Measure around the waist just below the belly button.

C - Hips - Measure around the hips at the widest point.
