Once you have completed your transaction, please come to Alief Taylor HS on September 12th from 8:00 a.m.- 8:25 a.m. to check in at the Camp. Dancing will begin at 8:30 a.m. Location: 7555 Howell Sugarland Rd. 77083 Main Gym. Find a Cadette Parent in a Red Top or Mrs. Dosal the Dance Director

This section releases the school district from liability. Please read carefully before signing.

I, the undersigned parent or legal guardian of the student named above, understand and agree that the Dance Clinic is a school-sponsored activity and function.

By signing below, I express my understanding that participation in dance activities involves inherent risks of physical injury, illness, or property damage. I voluntarily assume all risks associated with my child's participation.

I hereby release, waive, acquit, and forever discharge the school district, its school board, employees, students, agents, and sponsors from all liability, claims, demands, or causes of action. This release is intended to cover all injuries and illnesses of every name, type, kind, or nature, and personal property damage, if any, which may be sustained or suffered from any cause connected with, arising out of, or from participation in the listed events.
