I, the above- named Participant, have voluntarily agreed to participate in the above-described Program, which is associated with Pasadena Independent School District. I understand and agree that participation in this Program is completely voluntary and PISD employees are not required to participate in this Program as a condition of, or part, of their employment. All participants will be required to abide by safety rules that have been put into place by the Program. As with any competitive athletic activity, voluntary participation in the Program will expose participants to an inherent risk of injury despite the safety precautions implemented by the Program. Program participants are solely responsible for being self-aware of their abilities and fitness level. Participants are expected to take all necessary steps to ensure that they remain safe while participating in the Program.
In consideration of my participation in this Program, I hereby accept, and I hereby release Pasadena ISD from any and all liability for injuries, damages, or loss to my property and for any and all illness or injury to myself including death, that may result from or occur during my participation in this event. I understand my participation is voluntarily and assume all risks associated with the event. I further agree to indemnify and hold harmless Pasadena ISD and its governing board, officers, employees, and representatives from liability for the injury or death of any person(s) and damage to property that may result from my negligent or intentional act or omission while participating in the described above event.
All participants in the Program agree to allow their name and picture to be used for the promotion of this Program following the guidelines in place by Pasadena ISD.
I HAVE CAREFULLY READ THIS AGREEMENT AND UNDERSTAND IT TO BE A RELEASE OF ALL CLAIMS AND CAUSES OF ACTION FOR INJURY OR DEATH TO MYSELF OR DAMAGE TO MY PROPERTY THAT OCCURS WHILE PARTICIPATING IN THE DESCRIBED ACTIVITY AND IT OBLIGATES ME TO INDEMNIFY THE PARTIES NAMED FOR ANY LIABILITY FOR INJURY OR DEATH OF ANY PERSON AND DAMAGE TO PROPERTY CAUSED BY MY NEGLIGENT OR INTENTIONAL ACT OR OMISSION.
I certify that I as a Participant am over the age of 18 and have knowingly and voluntarily signed this Agreement.
