I understand that my student’s participation in ODC's Young Dancer Workshop, hosted by the OHS Department of Dance, is optional and at their own risk.
I hold OHS, ODC, and their officers, agents, and employees harmless for any and all liability in conjunction with this event and its individual activities. I recognize that OHS and ODC assume no responsibility for any accidents, damages, injuries, or other occurrences or losses of any kind associated with my student’s participation in this event.
I understand that travel both to and from ODC's Young Dancer Workshop is our responsibility, risk, and expense.
I hereby grant permission to the OHS Department of Dance to use, including publicly display, my student's photograph as part of promotional videos, the OHS Dance website, or in other official OHS or ODC publications without future notice. I acknowledge that OHS and ODC have the right to crop, edit, or modify the photograph(s) at their discretion.
I also understand that once my student's photograph is published on a website, it can be downloaded by any computer user. Personal information, such as a student's full name, parent's names, address, and telephone numbers will never be published. If a student's name is used with a photograph it will be in the form of a first name only. For example, student Jane Doe may be listed as "Jane."
Therefore, I agree to indemnify, defend, and hold harmless OHS, ODC, and their officers, employees, agents, successors, assignees, and members (the "Indemnified Parties") from and against any and all claims and liabilities resulting from this publishing.
