I authorize my child to participate in the Maricopa Unified School District activity, sport, event, camp, competition, program, or related activity identified above ("Activity"). I understand that participation is voluntary and that there are risks and dangers associated with participation. I understand and acknowledge that participation in the Activity involves risks of injury due to inherent dangers that cannot be eliminated regardless of the care taken to avoid them. These risks may include, but are not limited to:
• Physical contact with other participants, spectators, staff, or volunteers;
• Contact with playing surfaces, facilities, fixtures, equipment, or vehicles;
• Strenuous physical activity, exertion, running, jumping, lifting, or changes in speed and direction;
• Weather-related conditions, including heat, cold, wind, rain, and sun exposure;
• Travel to and from Activity locations.
Possible injuries may range from minor injuries such as cuts, bruises, strains, and sprains to serious injuries such as fractures, eye injuries, concussions, heat-related illness, heart-related conditions, paralysis, or death. I voluntarily assume all risks associated with participation in the Activity. In consideration for being permitted to participate in the Activity, I hereby release, waive, discharge, and covenant not to sue Maricopa Unified School District, its governing board, employees, volunteers, coaches, administrators, sponsors, agents, contractors, affiliates, and representatives (collectively referred to as the "Releasees") from any and all claims, demands, actions, causes of action, damages, costs, expenses, or liabilities arising out of or related to participation in the Activity, including personal injury, illness, property damage, or wrongful death, except as prohibited by law. I understand that photographs, video recordings, audio recordings, and other media may be taken during the Activity. I grant Maricopa Unified School District permission to use the participant’s name, image, likeness, voice, and demographic information for educational, informational, promotional, and public relations purposes in any media format now known or later developed without compensation. I understand that Maricopa Unified School District does not provide personal medical or health insurance coverage for participants. I acknowledge that I am responsible for any medical expenses incurred as a result of participation in the Activity. In the event of an injury, illness, or medical emergency involving the participant, I authorize Maricopa Unified School District personnel, agents, and representatives to obtain emergency medical treatment, hospitalization, transportation, surgery, anesthesia, or other medical care deemed necessary by licensed medical professionals. I agree to be financially responsible for any such treatment and release the Releasees from liability related to the provision of emergency medical care.
