I give my child,__________________,permission to attend the TAFE Area 4 Competition on Friday, November 13, 2024 and to ride the AISD Transportation to and from the Conference. I have paid the registration fee and will support and encourage them to complete/work on any competitive events they choose to compete in. (TBD)

*Medication Release

I give permission for Ms. Beacom to administer:

Headache medicine (Tylenol/Advil)

Stomach medicine (Gas X/Pepto)

Menstrual Relief (Midol/Pamprin)

I prefer Ms. Beacom to call before administering meds.

My child will have their own medication and self-administer as needed.

I will provide Ms. Beacom with additional information about medication for my child .
