*Please acknowledge and agree to the statement: I hereby give consent for my child to participate in PISD Intermediate Cheer Camp hosted by the Maverick Cheerleaders of Pasadena Memorial High School. I understand that injuries and property loss can occur during such a sport as cheerleading. I hereby release and agree to hold harmless PMHS coaches and cheerleaders and/or affiliated PISD coaches and any employees of PISD of any liability claims, demands, injuries, or causes of action which may arise by virtue of acts, or decisions to act, negligible or otherwise. Pasadena Independent School District does not carry insurance for summer fitness/recreation programs. Parents/Guardians will be responsible for any medical expense incurred. A parent/guardian MUST sign this form if child is under 18 years of age. The undersigned parent/guardian authorizes PMHS coaches/instructors, and employee's permissiont to request medical treatment as necessary to insure the well-being of the above listed dependent.
