Participation Waiver
Being fully aware of the inherent dangers associated with football and in consideration of your accepting our application, we hereby, for ourselves and our heirs, do waive and release any and all rights and claims for damages we may have against the American Fork High School Football Camp, its coaches, its sponsors, the Alpine School District, or their respective representatives for any and all injuries suffered at any activity associated with the program.
Be it known, that in the event I can not be reached, I, the undersigned parent or guardian of the participant named above, give and grant unto any Emergency Medical Technician, medical doctor, or hospital, my consent and authorization to render such aid, treatment or care to said participant, as may be required in the event of an injury, or similar emergency which occurs during any activity associated with this program. I also acknowledge and understand that any expense related to an emergency related to this program that is not covered by insurance will be my responsibility. I agree that expenses for such emergencies are not the responsibility of the American Fork High School, the coaches, the parent sponsors, or the school district or their representatives.
