By registering for this event you are agreeing to give the registered student permission to participate in the Cy-Fair Cheer Clinic as well as release and hold harmless the Cypress Fairbanks I.S.D. and Cy-Fair High School from liability for injuries incurred by my child while attending the above-mentioned activity. You hereby authorize emergency medical treatment for my child when I cannot be contacted to give personal consent. Such medical treatment may include administration of tests, drugs, X-rays, examinations, and anesthetics, dental or surgical examinations or treatment, and general hospital care as necessary. This activity is not related to or sponsored by the Cypress Fairbanks I.S.D. You also hereby consent and agree that Cy-Fair Cheer has the right to take or use any photographs of above student (and/or any property) and to use these in any and all media and media worldwide including online and social media, now or hereafter known, and for any purpose whatsoever.
