Field Trip Forms

Please note that students will need to return a signed Field Trip Agreement and Medical Waiver before attending the field trip. These forms will be sent home closer to the field trip date.

Lunch At the Farm

During the field trip, students will take time to enjoy a sack lunch. Students may bring a lunch from home or purchase a sack lunch from the cafeteria. All students should bring a large water bottle that can be refilled.

What to Wear

For everyone's safety and comfort, students, teachers, and parents must wear closed-toe shoes and comfortable clothing that can get dirty. This trip is rain or shine.

Chaperone Information

Parents and guardians who would like to attend the field trip as a chaperone must:

• Be an approved Community ISD chaperone. Background checks must be completed each school year.

• Complete the required CISD background check.

• Notify your child's teacher in advance that you plan to attend.

• Pre-pay the $12 chaperone admission fee on SuccessFunds

Please note that parents/guardians who have not completed the required background check and been approved as a district chaperone, or who have not notified the teacher in advance, will not be able to accompany students on the field trip and admission fee may be forfeit.
