Join us for the CYSA Summer Soccer Camp! Players will work on foot skills, passing, shooting, ball control, agility, and game play in a fun and encouraging environment. Open to players in U6-U16 based on Fall 2026 age divisions.

I acknowledge that the participant for which I am registering will participate in this event hosted by CYSA. I release CYSA representatives and volunteers from any liability for injury, death, or property damage that may occur during the event. I agree to indemnify and hold harmless from any such claims arising from the participant's actions during the event. I have read and understand this release of liability, and I agree to it freely and voluntarily. I certify my ability to do so on behalf of the participant.

In the event of an emergency, I authorize Canton Youth Soccer Association (CYSA), its coaches, staff, and volunteers to obtain emergency medical treatment for my child if I cannot be reached, including first aid or hospital care. I understand that every reasonable effort will be made to contact me or my emergency contact before treatment is provided. I accept responsibility for any medical expenses incurred as a result of treatment. I certify that my child is physically able to participate in camp activities and that I have disclosed any relevant medical conditions, allergies, or limitations.

I grant permission for Canton Youth Soccer Association (CYSA) to photograph and/or record my child during camp activities. I understand that these photos and videos may be used by CYSA for promotional purposes, including social media, websites, printed materials, and other marketing efforts. No compensation will be provided for the use of these images. If I do not wish for my child to be photographed, I will notify CYSA in writing prior to the start of camp.
