I hereby waive and release Avid Volleyball, Kent k Nitta, Scott Lee, Andrew Richards and Timpanogos High School, Alpine School District and their employees, Head Coach Jen Downey, her assistant coaches and volleyball clinic instructors from all liability for any injures incurred while participating in the volleyball camp for which my child is registered. I recognize the risks inherent in my child's participation, and I assume full responsibility for all injuries that may arise. I know of no mental or physical limitations that may affect my child's ability to safely participate in this camp. I authorize Jen Downey and Alpine School District and their representatives to act for me according to their best judgement in an emergency requiring medical attention. I also give permission for the above-named child to be transported to and receive medical treatment at a local medical facility, and I guarantee the payment of all expenses incurred from such transportation and treatment.
