Dear Parents/Guardians,


GOOD NEWS! We have the opportunity to attend The Little Mermaid Matinee on Monday, November 16th at Desert Hills High School. We will get on the buses at 10:00 and return to school around 1:10 for lunch. Please mark below if your child will bring their own lunch or if they will need to purchase a school lunch. We have to give our headcount asap, at the latest by October 4.


We are requesting a $2.00 donation to cover the cost of seats. However, donations are entirely optional, but any amount is greatly appreciated. Please have your student bring any donations directly to the school office if you would like to. Thank you for your consideration!

I acknowledge that the participant for which I am registering will participate in this event hosted by the school. I release the school and district, its agents, employees, representatives, and volunteers from any liability for injury or property damage that may occur during the event. I agree to indemnify and hold harmless from any such claims arising from the participant's actions during the event. I have read and understand this release of liability, and I agree to it freely and voluntarily. I certify my ability to do so on behalf of the participant.
