I approve of my child’s attendance at camp and certify they are in good health and able to participate in the program of activities. I further release the coaches, players, volunteers, Jerome School District #261 and their employees, agents and assigns from any and all liability that may arise out of my child’s participation in the camp. I acknowledge that I am responsible for any and all medical charges incurred in connection with my child’s attendance at the Lady Tigers Volleyball Skills Camp. I also give permission for my child to be included in video/photo publications for Jerome High School.
