I authorize my child to use tools and materials including but not limited to: glue guns, scissors, and batteries. I hereby agree to release the Parkview Technology Student Association, its representatives, officers, and volunteers from liability for any injury to my child while attending Parkview TSA STEM Academy After School Program.
I permit Parkview TSA to use video footage and photographs of my child for publicity that might include but is not limited to: website, PowerPoint presentations, promotional videos, flyers or news publications.
I acknowledge that the participant for which I am registering will participate in this event hosted by the school. I release the school and district, its agents, employees, representatives, and volunteers from any liability for injury, death, or property damage that may occur during the event. I agree to indemnify and hold harmless from any such claims arising from the participant's actions during the event. I have read and understand this release of liability, and I agree to it freely and voluntarily. I certify my ability to do so on behalf of the participant.
