In order to participate in Scare a Teacher, you or your team are required to pay a 5 dollar entry fee. This fee is non-refundable and will help fund the event and the winning prizes.

If you have any additional questions, talk with a Sophomore Class Officer or Miss Shaw in room 220.

Come up with a name to represent your team in the compeition. If you are an individual, you can just put your name instead.

Please list all the people participating in your group. If you are participating as an individual, just list yourself.

By selecting yes you and/or all participates in your group are agreeing to the following statement:

I agree to only scare my assigned teacher each week. The "scare" will only take place during the approved times (before school, after school, and lunch) and days (Wednesday and Thursday.) The "scare" will be school appropriate and not leave a mess or cause harm to any individual. I will not touch the teacher I am assigned to scare. I understand that the teacher may use a swear word/swear while being scared and am willing to subject myself to hearing said word(s) if that is the case. I will ensure to film the "scare" and send it to Miss Shaw (amy.shaw@ccsdut.org) by the deadline each week (Wednesdays at 5pm) I participate in. If it is reported that I break any of these rules, I understand I will be disqualified from the competition and will not receive a refund for the participation fee.
